• • Visit listed Unipath Specialty Laboratory branches in Ahmedabad and show your Unifit Diabetes Club Card or mention your unique ID number.
• • Reception will verify membership ID number and validity.
• • Avail your diabetes tests

Terms & Conditions:

• • This is a Unipath Specialty Laboratories diabetes club card and would only be accepted at listed Unipath Specialty Laboratories in Ahmedabad at the time of lab/Home visit.
• • The diabetes club card has validity of 1 year from the date of issuance after which the club card membership can be renewed at FREE of cost
• • Unutilised test packages can not be carry forwarded after the membership validity is over
• • This is a one person-one card scheme. The person who has the diabetes club card is deemed to be the beneficiary.
• • More than one diabetes club card cannot be used in one transaction.
• • The Unipath Specialty Laboratory diabetes club card CANNOT be used in conjunction with other existing offers/promotions.
• • Home visit charges are not included in Unipath Specialty Laboratory diabetes club card test packages. Regular Home Visit charges will apply on demand.
• • Any dispute related to the Unipath Specialty Laboratory diabetes club card should be referred to the issuing company and the decision of the issuing company shall be final. In case of any legal disputes, the courts at Ahmedabad shall have the exclusive jurisdiction.
• • Unipath Specialty Laboratory Limited reserves the rights to modify diabetes club card schemes / offers without any prior notice to diabetes club card members.
• • Unipath reserves the right to deny accepting diabetes club card, if it suspects that there is duplicity of club card.
• • If a diabetes club card gets blocked on account of technical issue, it would get enabled in 72 hours.
